The Broome Regional Aboriginal Medical Service (BRAMS) is an Aboriginal Community Controlled Health Service which has been caring for the Broome community for more than 45 years. When we first opened our doors in 1978, BRAMS was the first remote Aboriginal Medical Service in Western Australia.
We deliver comprehensive, holistic and culturally responsive primary health care, social and emotional wellbeing services, and NDIS support to more than 6500 Aboriginal and Torres Strait Islander people in the Broome region.

About the Role
Are you passionate about supporting Elders and older Aboriginal and/or Torres Strait Islander people to access the care they deserve? As a BRAMS Aged Care Connector, you’ll play a vital role in ensuring our community understands and benefits from their aged care entitlements.
This role will involve.
Raising awareness of available aged care services and support options.
Ensure services are organised, well structured, and fully supported for each client.
Coordinate care pathways by connecting clients with both BRAMS and external aged care services to ensure they receive the right care at the right time.

Desired Skills and Experience
About you
To be successful in the role you will have demonstrated experience in Aged Care or Disability Services.
Proven background in delivering support and services in aged care or disability settings.
Strong connections with the local Aboriginal and Torres Strait Islander community, with a sound understanding of the Aboriginal Medical Service Model of Care.
Excellent communication and interpersonal skills, with the ability to build trust and foster strong relationships within the community.
Understanding and application of person-centered care principles and trauma-informed practice to support clients effectively.
Experience working with older people and an eagerness to learn about the aged care landscape and internal ACCHO services available to clients.
